My 1992 pajero 2.5td lwb cuts out

Hi my pajero 1992 2.5td lwb cuts out on ode long and also when you come to stop at a junction and traffic lights it judders before cutting out. It is hard to start cold which I am replacing heater plugs for but not sure what is causing the rest
